Youth who are transitioning out of state custody are supported by a case manager to achieve independent, self-supporting lives in their communities as they enter adulthood. Youth ages 15 to 22 are eligible for supports in this program.

In 2008:

• 49 youth received support in this program
• 16 youth received assistance finding safe, affordable housing
• 15 youth accessed educational and/or job training programs
• 26 were enrolled in health care coverage
